How it works

Does massage improve circulation?

Skin blood flow goes up and that is real. Muscle blood flow does not. Moving the limb yourself does roughly three times more than any massage technique.

This one is half true, which is why it survives. The half that is true is visible on the skin. The half that is claimed is about muscle, and it does not happen.

The short version

  • Muscle blood flow is not increased by massage. A study designed to test this is titled "Failure of manual massage to alter limb blood flow".1
  • A brief voluntary contraction raises it roughly threefold. Moving beats being moved, by a wide margin.1
  • After hard exercise, massage reduced blood flow compared with simply resting.2

What the study found

In 1997 researchers used Doppler ultrasound to measure brachial and femoral artery blood flow while applying massage. The title states the result: Failure of manual massage to alter limb blood flow.1

No massage technique altered blood flow in either the arm or the leg. In the same study, a brief voluntary muscle contraction raised femoral artery flow from 371 to 1,087 mL per minute. The participant contracting their own leg for a moment did roughly three times more for circulation than anything the therapist's hands could do.

A separate 2010 study looked at the situation people usually have in mind, which is after strenuous exercise. It found forearm blood flow was higher at rest than during massage, at 766 mL per minute against 540.2 Pressure on a limb mechanically impedes flow while it is being applied, which is not a surprising result once it is stated plainly.

What does change

Skin blood flow. The flush, the warmth, the pink patches after firm work: that is a real vascular response in the superficial tissue, and it is honest to describe it as such.

The slippage happens when that visible skin response is used as evidence for something happening several centimetres deeper in muscle, where it was measured and did not occur.

There is also a real biological recovery effect, just not a circulatory one. Muscle biopsies after damaging exercise showed massage reduced inflammatory signalling and promoted mitochondrial growth.3 That is a cellular response to mechanical load, not a plumbing effect.

What this means in practice

If your goal is circulation specifically, whether for cardiovascular health, swelling in the legs from sitting, or recovery between training sessions, then walking, cycling and calf raises are supported by the measurement in a way that a massage table is not. This is the honest advice even though it points away from a booking.

Massage is worth having for what it does do, which is set out on the how massage works page: a nervous system response that reduces pain sensitivity, a measurable reduction in inflammatory signalling, and the largest effects in the whole literature on anxiety and mood.

Lymphatic drainage is a separate question with a separate evidence base. It is a distinct technique aimed at lymph rather than blood. Its evidence is strongest for lymphoedema after breast cancer treatment, where it adds roughly 7% volume reduction on top of the 30% to 38.6% achieved by compression garments alone.4 For post-surgical use it is weaker: after total knee replacement, the highest level of available evidence does not support it.5

Its evidence is strongest for lymphoedema following breast cancer treatment, where it adds roughly 7% volume reduction on top of what compression garments achieve on their own. After total knee replacement, the highest level of evidence does not support it.
